This guide is designed to help physicians and clients navigate the process of authorizing and arranging Non-Emergency Medical Transportation (NEMT) through Medicaid. By following these steps, you can ensure a smooth experience when obtaining the necessary travel vouchers for Medicaid-covered travel.

Step 1: Travel Voucher Forms and Log Sheets

When arranging Medicaid transportation, you will need several key documents:

Travel Voucher Fill-In Template Instructions

  1. Complete the Voucher Template: Enter the necessary information into the Travel Voucher Fill-in Template.

  2. Load Voucher Paper: Load the voucher paper into your printer before printing the filled-in vouchers.

  3. Print Vouchers: When printing, ensure the "Comments & Forms" option in your print window is set to "Form Fields Only." If you do not see this option, contact your IT department for an update.

Travel Voucher Log Sheet

  • Use the Transportation/Accommodation Voucher Request Log spreadsheet to keep a detailed record of all vouchers issued. This record must be submitted when requesting additional vouchers.

Step 2: Ordering More Vouchers

When additional vouchers are needed:

  1. Fill Out the AK04 Travel Voucher Order Request Form: Complete this form, providing all necessary details.

  2. Submit the Log Sheets: Ensure that your completed log sheets are included with your request for new vouchers.

  3. Return Unused Vouchers: If you have unused vouchers, list the range of control numbers (e.g., 4894801-4895000) when submitting your order.

Recommendation: Alaska Medicaid Fiscal Agent HMS advises providers to request additional vouchers at least four weeks before they are needed to allow sufficient processing and mailing time.
